Position Summary:

The Licensed Practical Nurse provides general nursing care to patients, families and caregivers in the Medical Group practices.  The Licensed Practical Nurse assists and advocates for the patient and families health in support of the patient centered approach and delivery model of care, and is an active care team member.  Responsible for collaborating with physicians/ providers , registered professional nurses and multidisciplinary care team members.  A Licensed Practical Nurse can perform tasks and responsibilities within the framework of case finding, health coaching, and supportive care under the direction of the registered professional nurse or licensed physician or licensed health care provider.  Nursing shall perform in accordance with the standards outlined for Licensed Practical Nurse in the NYS Education Law, NYS Office of the Professions, and  by the State Nurse Practice Act.  A Licensed Practical Nurse  will follow the mission, vision, I-CARE values, policies and procedures of Mohawk Valley Health System. 

Some Responsibilities Include:

  • The licensed practical nurse delivers nursing care services to patients in accordance with New York State nurse practice act, regulatory standards and MVHS policies, practices and procedures, including but not limited to:  Establishing personal rapport with patients, families and care givers and understanding of plan of care throughout the patient’s experience;  Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, families and care givers;  Ability to gather patient measurements and collect patient data; Recording and reporting patient findings; Prepare patient for patient examination and treatment; Assists with examination and treatments; Performs tasks within the framework of supportive care, under the clinical direction of licensed health care provider or registered professional nurse; Supports self-care , self- management and the established plan of care. Assure quality care by adhering to standards measuring health outcomes against patient care goals and standards;  Follow the MVHS and Medical Group patient centered care model and standards of care set by the State Board of Nursing, state nursing practice act, and other governing agency regulations.
  • The licensed practical nurse provides for patient safety, environment of care processes and care services to patients in accordance with New York State nurse practice act, regulatory standards and MVHS policies, practices and procedures, including but not limited to:  Protect patients, families , caregivers, employees by adhering to infection control policies and protocols, medication administration and controlled substance regulations; Utilize standard precautions and practice at all times;  Maintain equipment, supplies, and inventory as appropriate; Maintain a safe and clean work environment by complying with procedures rules, and regulations; utilize safety devices at all times.
  • The licensed practical nurse provides for patient safety, performs nursing care safely, and environment of care processes and care services to patients in accordance with New York State nurse practice act, regulatory standards and MVHS policies, practices and procedures, including but not limited to:  Document patient care service by charting in patients’ medical records accurately and timely in accordance with licensure, regulatory standards; Maintain continuity of care with the practice care team by communicating plan of care and ensuring that patient administration of care delivered is directed by the licensed health care provider (physician/provider or registered professional nurse); Maintain patient confidentiality and a private environment; Maintain and anticipates needed supplies, equipment and medications verifying receipt utilizing MVHS ordering procedure.
  • The licensed practical nurse provides for safe nursing care, provides for patient safety and environment of care processes and care services to patients in accordance with New York State nurse practice act, regulatory standards and MVHS policies, practices and procedures, including but not limited to:  Maintain technical knowledge by attending educational workshops, mandatory in-services; reviewing publications and participating in department committee meetings or work groups. 
  • The licensed practical nurse participates in the practices quality improvement activities, including but not limited to:  Engage in quality improvement initiatives including continuing education, participation in system-wide and department specific teams; Maintain a cooperative relationship among the healthcare team by effectively communicating information; timely responding to requests; building rapport; participating in team problem solving and team building with all care team members.
  • The licensed practical nurse provides nursing care safely and environment of care processes and care services to patients in accordance with New York State nurse practice act, regulatory standards and MVHS policies, practices and procedures, including but not limited to:  Provide service excellence to patients, their families and caregivers that meets or exceeds patient expectations through offering empathetic and compassionate patient-centered care; In collaboration with all the practice care team members has the ability to assist patients, families and care givers needs and concerns using positive service recovery methods; Respond to patient requests, communicates effectively to the licensed health care provider who can fulfill the patient request; Provide culturally sensitive care.

Requirements:

  • Graduate of an accredited school of licensed practical nursing program
  • Bachelors or Masters of Science in Nursing from accredited nursing program preferred
  • Current New York State licensure as a Licensed Practical Nurse
  • Basic Life Support Certification - Must receive BLS certification within 6 weeks of hire date
  • One (1) or more years of experience in health care setting
  • Minimum of one (1) year of experience in Medical Office Practice, Outpatient Setting or Specialty preferred
