Admissions Coordinator - Full Time - Days

Job Summary

The Admissions Coordinator provides a point of entry and is responsible for coordinating all admissions into the sub-acute unit of the MVHS RNC.  In collaboration with the interdisciplinary team, plans, implements and evaluates patients for admission.  Serves as a liaison between the RNC and the Wynn hospital, as well as other area facilities.

Core Job Responsibilities

  • Liaises with hospital staff to obtain referrals or leads on potential admissions for sub-acute care. Reviews all referrals for sub-acute care.  Coordinates admissions reviews and processes with nursing, pharmacy, insurance/payor or finance to ensure a smooth transition from acute care into the facility.
  • Ensuring admissions processes and insurance/finance arrangements and documentation are accurate, complete and in place.
  • Relays any special information or instructions from the hospital to the RNC to ensure a smooth transition of services upon arrival.
  • Keeps current on census within the Sub-Acute unit.
  • Participates in patient/family meetings.
  • Participates in discharge planning processes as needed.
  • Maintains collaborative relationships with MVHS departments and staff as well as other health-related facilities.  Promotes a positive image of the facility in all interactions. May give tours of the facility to families or members of the local community.  
  • Treats patients/residents and their families with respect and dignity. Respects and protects residents’ rights.
  • Keeps supervisor informed of any concerns or problems. 
  • Performs related duties as directed.
     

Education/Experience Requirements

REQUIRED:

  • Bachelor’s degree in Human Services, Social Work, Psychology or similar field, or equivalent work experience. 
  • One year experience in a hospital or nursing home.  
  • Organized, detail oriented and able to multitask.
  • Excellent interpersonal, verbal and written communication skills. 
  • Proficient with MS Office and able to learn new software (EMR).
  • Ability to travel to other facilities within the area. 
     

PREFERRED:

  • Experience in a long term care environment.
  • Familiarity with EHR/EMR software.
