Job Summary

The Certified Diabetes Care and Education Specialist prepares and delivers education and coaching for patients with diabetes, prediabetes and their families or caretakers.  Promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.  Works as part of an interdisciplinary team.

Core Job Responsibilities

  • Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes into clinical practice. 
  • Completes comprehensive assessments for each patient including emotional and behavioral health and interprets personal health data. 
  • In collaboration with physicians or other care providers, develops an individualized care and education plan based on the patient’s assessed needs and goals. 
  • Educates patients on their diabetes, prediabetes or cardiometabolic diagnosis and provides guidance on appropriate self-management.  
  • Trains patients or their caregivers to check blood glucose levels, administer insulin, and identify and respond to hypoglycemia. Provides nutritional counseling, meal planning, and weight control advice meant to assist patients in managing blood glucose levels.
  • Actively participates in the quality improvement (QI) process and adapts practice/process based on QI findings.
  • Contributes to research and applies current research and evidence-based care to practice.
  • Applies the AADE7 self-care behaviors to educate on and initiate behavior change.
  • Engages in lifelong learning and serves as a role model of professionalism.
  • Collaborates with community partners to establish and maintain on-going support options. 
  • Performs related duties as assigned.
     

Education/Experience Requirements

REQUIRED:    

  • Bachelor's degree in nursing.
  • Is flexible and able to work with patients in a remote telehealth capacity for day and evening sessions or appointments using a reliable and strong internet connection.
  • Ability to work as part of a team. 
  • Excellent interpersonal, verbal and oral and written communication skills. 
  • Problem-solving skills. 

PREFERRED:

  • Master's degree in Nursing or related field.

Licensure/Certification Requirements

REQUIRED:    

  • Registered Dietitian (R.D.) with certification by the Commission of Dietetic Registration (CDR) and NY CDN (Certified Dietitian-Nutritionist).
  • Certified Diabetes Care and Education Specialist (CDCES) or obtained within 2 years of hire.  If not certified, 2 or more years of experience working with people with diabetes.
  • Certification as National DPP Lifestyle Coach or willing to obtain within 1 year of hire.
