CLINICAL NURSE COORD - RNC
Job Summary
The Clinical Nurse Coordinator (CNC) – RNC is a dynamic role that ensures and delivers high quality, patient-centered care and coordination of all clinical functions in the sub-acute rehab unit during the designated shift. In collaboration with interdisciplinary team members and under the direction of the Nurse Manager, the CNC directs, monitors and evaluates nursing care in accordance with established policies/procedures, serves as a resource person for staff, and models a commitment to the organization’s vision/mission/values. The CNC provides unit-based leadership and education while engaging staff and promoting evidenced based practice supporting quality outcomes, cost effective performance and organizational changes.
Core Job Responsibilities
- Coordinates assignments and oversees the completion of assigned tasks for nursing team on the unit in collaboration with the Nurse Manager.
- Creates an environment of care delivery and teamwork that supports improved outcomes and patient service in collaboration with subject matter experts and other managers.
- Coordinates interventions and plans of care for patients with the interdisciplinary team. Evaluates care plans for effectiveness and recommends revisions as indicated by the patient’s response to treatment.
- Ensures physician orders are noted and carried out timely.
- Takes assignment to provide direct resident/patient care when indicated or assigned.
- Coordinates labs, appointments, and special instructions for all patients.
- Notifies physician of changes in patient’s condition and follows through until appropriate action is taken.
- Provides thorough reports to oncoming shift. Follows through on information given by shift report, patient/resident or family regarding patient/resident concerns.
- Assesses and reassesses patient/resident’s pain. Utilizes appropriate pain management techniques. Educates the patient/resident and family regarding pain management.
- Submits written documentation in a timely, clear, concise and pertinent manner.
- Acts as a preceptor/resource person and assists others in implementing nursing care in a manner that contributes to the excellence of the nursing department and maximizes teamwork.
- Keeps leadership informed of any issues, problems or concerns timely.
- Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patient/residents and their families.
- Advocates for patient/resident’s rights.
- Promptly responds to or escalates complaints or investigations.
- Promotes and role models a culture of service excellence and customer service in all interactions throughout the unit.
- Ensures quality nursing care is rendered to all patients in accordance with the New York Nurse Practice Act, National Standards of Practice, and Nursing and Clinical Service stands of care and practice.
- Understands and complies with regulations, MVHS and divisional policies and procedures.
- Participates in orientation of new staff and performance evaluations of nursing staff.
- Follows proper infection control procedures at all times.
- Remains calm in emergency or stressful situations.
- Performs other related duties as assigned.
Education/Experience Requirements
REQUIRED:
- One year of nursing experience in a hospital, rehabilitation or skilled nursing home.
- Demonstrated ability to lead, guide and motivate others.
PREFERRED:
- BSN preferred.
- Prior supervisory or lead experience.
- 4 years of nursing experience.
Licensure/Certification Requirements
REQUIRED:
- Registered Nurse with licensure in NYS.
- BLS certification.
