Job Summary

The Community Benefits Coordinator is responsible for the coordination of community and environmental benefits available for the MVHS patients. This position will complete evaluations to determine patient’s eligibility for community resources. Duties include interfacing with community agencies and members of patient’s care team to connect patients to supports and resources necessary to ensure optimal functioning (ensure quality, efficient comprehensive care management for our diverse population). 

Core Job Responsibilities

  • Direct responsibility for evaluating, implementing, and coordinating community resources for patients.
  • Completes assessment interviews to evaluate the social and environmental factors related to the patient’s health problems and identify barriers to care.
  • Acts as an intermediary between patient’s cancer center team/medical providers and other community agencies, assisting patients to locate and utilize appropriate health system services.
  • Completes coordination of services, community resources, and documentation per New York State (NYS) Department of Health (DOH) regulations/agency policies.
  • Collaboration and consultation with the patient’s physician, family, and other care managers to assist them in understanding the significant social and environmental factors related to the patient’s diagnosis and psychosocial problems for development of patient services.
  • Completes community referrals or required documentation for community resources; maintains appropriate records and submits paperwork in a timely manner. 
  •  Facilitates coordination of care by managing referrals for patients receiving care within the Mohawk Valley Healthcare System (MVHS).
  • Evaluate, in conjunction with the Financial Coordinator, financial and insurance needs for patients and assist patients in required paperwork for programs/ services to address identified needs.
  • Coordinate and facilitate, in conjunction with the Financial Coordinator, programs and resources to address psychosocial needs of patient.
  • Community Benefits Coordinator in conjunction with the Financial Coordinator provides referrals for transportation resources and educates on how to utilize these resources.  When appropriate coordinates with office staff to arrange medical transport for patients.
  • Conducts practices within the team concept to ensure a high quality of professional service and maintenance of MVHS/ICARE standards and policies.

Education/Experience Requirements

Required:

  • Associates Degree in Social Work, Human Services, or related field.
  • Knowledge of Medicaid and Medicare eligibility regulations.
  • Ability to work independently and meet deadlines.
  • High level of computer literacy including, but not limited to, data entry, retrieval, presentational and report generation.
  • Ability to work with a socially and culturally diverse patient population, presenting diverse physical conditions, and in a variety of settings (ie. Radiation Oncology, Outpatient Infusion, Survivorship and medical office ).
     

Preferred:

  • Two years experience in community services and/or case management.
