LPN - NYSUT (VNA)
Job Summary
The Licensed Practical Nurse - NYSUT (VNA) is responsible for performing tasks and responsibilities within the framework of health teaching, health counseling, and provision of supportive and restorative care as directed by the Registered Professional Nurse.
Core Job Responsibilities
- Assists in provision of nursing care to the home health patient. Collaborates with the patient's primary case manager and other members of the health care team. Makes and documents home care nursing visits.
- Gathers data and reports significant change in patient's condition to Registered Nurse or Supervisor as appropriate promptly.
- Instructs patients and caregivers according to the plan of care, implements orders for medications and treatments as prescribed by the physician.
- Promotes good health practices, helping the patient in accepting and assuming responsibility for attaining the highest level of independent functioning possible.
- Completes, maintains and submits accurate, legible, and relevant nursing documentation using established agency abbreviations regarding the patient's condition and all care and treatment given.
- Monitors patient expectations and satisfaction; participates in patient care conferences as appropriate.
- Accepts assignments and performs duties within the limits of preparation and experience, seeking and obtaining sufficient information necessary to function effectively in the home health care setting.
- Assumes responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education, in-service programs, professional conferences and community and agency committees.
- Participates in appropriate agency meetings and the quality improvement and utilization review activities of the organization.
- Supports the Agency's Compliance Program, ensuring that all information is kept confidential and is respectful of the Patient's Bill of Rights.
- Complies with all MVHS and VNA policies and procedures, as well as applicable regulations.
- Is flexible to work a varied schedule based on patient needs.
- Performs other duties as required.
Education/Experience Requirements
REQUIRED:
- At least two years of experience working as a Licensed Practical Nurse, preferably in acute care setting.
- Excellent written, verbal and interpersonal communication skills.
- Proficient with EHR tools.
- Ability to travel to patient homes which may not be handicap accessible with reliable transportation.
Licensure/Certification Requirements
REQUIRED:
- License and current registration to practice as a Licensed Practical Nurse in New York State.
- Current BLS certification.
