Job Summary

Under the clinical direction and administrative supervision by the Behavioral Health Supervisor/Director of Behavioral Health, responsible for providing individual and group therapy, psychological/social service, and discharge planning function for the inpatient psychiatric unit. Provide counseling and aftercare mental health services for patients as ordered by physicians. Completes concurrent insurance reviews to ensure reimbursement. Collaborates with the physicians for safe discharges. Ensure proper services and education are in place to accomplish this goal. May have to rotate in weekend coverage with both Emergency Departments for mental health evaluations on a limited basis. Make referrals an appropriate combination of short-term and group therapy approaches to resolve or ameliorate symptomatology. Completes required medical record documentation on a timely basis; and participates in the Quality Management Plan for the inpatient/outpatient services. Completes necessary continuing education requirements to ensure credential is in good standing. Works in a team environment.

Core Job Responsibilities

  • Assessment, diagnosis, intervention, and treatment planning for pediatric, adolescent, adult, and geriatric patients.
  • Performs initial behavioral health assessments on new patients.
  • Develops mental health diagnosis for pediatric, adolescent, adult, and geriatric patient’s using the most current edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5).
  • Makes recommendations for appropriate age specific and illness specific treatment interventions (plan) for each patient to the ED provider and documents on designated forms.
  • Performs a discharge summary on all patient providing criteria.
  • Provides for referral to other behavioral health and/or medical providers as necessary during the course of treatment.
  • Develops discharge plans for services which cannot be performed within the outpatient mental health service.
  • Coordinates referrals for assessment when symptom identification and behaviors indicate psychopharmacological treatment.
  • Make referral for interventions appropriate to age group and identified illness to resolve or ameliorate symptomatology.
  • Performs patient record documentation and maintenance as per Health System’s policy and procedure.
  • Contacts internal MVHS and external providers/agencies to gather medical record information to assist with treatment planning and ensures that patient completes a “Release of Information” for each request.
  • Attends and participates in Health System’s and/or community related activities.
  • Assumes on-call duties for designated periods of time and completes all paperwork as per policy.
  • Contacts insurance companies to obtain benefits and authorization for treatment and appeals if necessary; ongoing insurance reviews for inpatient.

Education/Experience Requirements

Required: Master’s Degree in Social Work from Accredited Graduate Program

Licensure/Certification Requirements

Required: NYS LMSW License within 6 months of hire date required; Strong organizational and communication skills
