Job Summary
Under the direction of the Therapy Manager, the Speech Language Pathologist is responsible for providing speech therapy to the patients assigned to him/her in a comprehensive manner promoting quality, continuity, and safety.
Core Job Responsibilities
- In collaboration with the physician, develop a therapy plan based on the evaluation and the goals established.
- With other personnel from the agency and with the physician’s approval, assist in the development, implementation, and adjustment of the patient care plan, for total patient care, including and considering patient goals and planning for discharge.
- Participate in patient care conference with staff of the agency, other community agencies and/or individuals who might be involved in the total rehabilitation of the patient.
- Observe, record and report patient reactions to treatment and any changes in patient condition, to the physician and the agency.
- Instruct other health team personnel including, when appropriate, home health aides, in certain phases of speech therapy with which they may work with the patient.
- Instruct the family on patient total speech therapy program.
- Participate in in-service education programs for agency staff.
- Act as a consultant to other agency personnel.
- Completes, maintains and submits accurate and relevant required paperwork regarding the condition of the patient and all care and treatment provided.
- Monitor patient expectations and satisfaction.
- Accepts assignments and performs duties with the limits of preparation and experience, seeking and obtaining sufficient information necessary to function effectively.
- Assumes responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education in-service programs, professional conferences and community and agency committees.
- Participates in appropriate agency meetings and the quality improvement and utilization review activities of the organization.
- Supports the Agency’s Compliance program, ensuring that all information is kept confidential and is respectful of the Patients’ Bill of Rights.
- Strives for a safe environment by adhering to the Agency’s Safety Program.
- Supports principles of the C.A.R.E. Program.
- Performs other duties and assigned by Therapy Manager.
- Responsible for compliance with HIPAA regulations.
Education/Experience Requirements
Required: Master’s Degree in Speech Language Pathology.
Licensure/Certification Requirements
Required: License and current registration to practice in New York State as a speech language pathologist; Certificate of Clinical Competency from the American Speech and Hearing Association or meet the requirements for such certification
