Job Summary

Ensures safe and effective use of medications through their proper storage, handling, evaluation, preparation, compounding, labeling, dispensing and administration. Through an in-depth knowledge regarding drugs and their uses: interprets and evaluates prescriptions for medications; selects compounds, labels, and dispenses medications; creates and maintains medication records; provides information and education regarding the safe and effective use of medications; supervises others in the proper storage, handling, preparation, and labeling of medications.

Core Job Responsibilities

  • Thoroughly evaluates rationality and appropriateness of order or prescription
  • Profiles drugs to be dispensed to satisfy requirements of prescribers' orders
  • Continually reviews patient allergy and hypersensitivity, to minimize the possibility of adverse drug reactions
  • Participates in the Pharmacy Clinical Consultation Program providing guidance and information for physicians, nurses and other professionals
  • Through ongoing review of patient medication therapy optimizes therapy by adjusting drug administration schedules, facilitating the transition from IV to oral therapy, and assessment of the effectiveness of drug therapy regimens
  • Consults with physicians and nurses concerning improvements in drug therapy regimens.
  • Compounds, labels, and dispenses drugs according to prescriber’s orders
  • Compounds and prepares sterile drug products (including chemotherapy) and formulations, using an in-depth knowledge of sterile technique and use of specialized equipment and supplies, such as laminar flow hoods, needles, syringes, and transfer devices
  • Reviews and approves all medications prepared by supportive personnel prior to dispensing including (but not limited to) patient medication cassettes and IV batch preparation
  • Transcribes prescribers’ orders into Pharmacy computer
  • Assists in assuring complete and accurate records pertaining to the receipt, distribution, and disposition of controlled substances
  • Assists in assuring complete and accurate records of special drugs, drug formulations, or compounded items
  • Appropriately assigns work to technicians and interns within the limits of their duties
  • Provides orientation to new employees regarding departmental policies and procedures

Education/Experience Requirements

Required: 5 year BS in Pharmacy
Preferred: 1-2 years Hospital Pharmacy

Licensure/Certification Requirements

Required: Current New York State RPh License or license eligible.
