Job Summary

The Clinical Stroke Data Abstractor will demonstrate an attention to detail, accuracy in abstracting clinical information and proficiency in the required data abstraction tools. Performs data abstraction, analysis and reporting of quality indicators. The individual must have the ability to accurately interpret data definitions and communicate both verbally and in writing the variances to key facility leaders.  This individual will enter clinical data or information into electronic data bases, or registries, or other abstraction software. 

Core Job Responsibilities

  • Performs retrospective review and in-depth analysis of identified medical records for stroke measure outcomes, regulatory data reporting and other focused data collections as assigned. 
  • Performs detailed, concentrated work with limited supervision, maintains an accurate knowledge base of the Stroke Measures and the outcomes to be achieved. 
  • Reviews, abstracts, collects and submits data for all hospital stroke Measures from patient’s medical records following the specifications of DNV and AHA/ASA standards to ensure compliance with nationally recognized quality patient care processes and clinical outcomes.
  • Monitors results and recommends changes for improved compliance. Prepares and presents Stroke Measure reports to Nurse Managers, Physicians and Directors as well as provides consultation.  
  • Provides education related to externally reported Stroke Measures.
  • Prepares an analysis report summarizing OFI’s.  Identifies opportunities for improvement (OFIs) based on Stroke Measure results. 
  • Provides reports from data collection as assigned. 
  • Works closely with all Stroke Program Department staff members as appropriate.  Demonstrates an ability to be flexible, organized in meeting the needs of the department, with changes in the workload and able to function under stressful situations. 
  • Communicates appropriately and clearly to physicians, administrative team, nursing staff, department staff and other healthcare team members.   
  • Adheres to and enforces the confidentiality policy of the department affecting the use and transmittal of all stroke program materials and patient information in registries.  
  • Assists with community activities to interact with public and educate on signs and symptoms and risk factors
  • Keeps current in best practices related to the role.  
  • Performs related duties as assigned.

Education/Experience Requirements

REQUIRED:

  • Minimum of five (5) years’ experience in an acute care setting.
  • Individuals must possess the strong written and verbal communication skills. 
  • Ability to multitask and work in a high pressure environment. 
  • Familiar with medical terminology and basic medical record design and requirements. 
  • Proficient with computer applications, to include the navigation of several electronic databases (MIDAS, EPIC, Excel, and Word). Data accuracy and timeliness will be validated by another stroke team member.
  • Must have and demonstrate strong organization and planning abilities.

 

Licensure/Certification Requirements

REQUIRED:

  • Current NYS RN license.

PREFERRED:

  • BSN or BS with a major in Nursing
