Job Summary

The Stroke Program Clinical Coordinator responsible for ongoing standardization of clinical practice and competency in the care of stroke patients.  Coordinates activities of nursing, medicine and ancillary services in providing evidences based care.  Coordinates activities to achieve and maintain DNV certification as a stroke center.  Serves as a liaison and resource to all departments to facilitate quality patient care.  Participates in performance improvement activities that decrease length of stay, including promotion of use of standard stroke orders sets and appropriate use of thrombolytics.  Promotes internal and external awareness of stroke activities by liaison with ENT, community resources, conducting stroke screening and stroke support groups.  Is responsible for coordinating all aspects of education relating to stroke care and treatment, including patient education materials.  Serves on regional stroke committees.  Responsible for developing, conducting and evaluating education related to Quality/ Performance Improvement initiatives. 

Core Job Responsibilities

  • Collaborates with subject matter experts and other managers to create an environment of care delivery and teamwork that supports improved outcomes and patient service for stroke patients.
  • Educates team members in theoretical and clinical competencies.
  • Assists in the implementation of clinical pathways, standards of care, and guidelines for integrated patient care.
  • Participates in program development and program growth, working as a liaison between nursing staff, providers, patients and the community.
  • Ensures quality nursing care is rendered to all patients in accordance with program and regulatory requirements
  • Collaborates with unit leadership to achieve unit and organization quality improvement and outcome-based initiatives.
  • Attends meetings and conferences to exchange information on trends and best practices in the area of specialization or unit.  
  • Perform other duties as required.

Education/Experience Requirements

REQUIRED:

  • Five (5) years experience in acute care nursing, emergency department or critical care hospital experience.  
  • One (1) year teaching/management experience at unit level.  
  • Excellent communication skills, both written and oral.
    Organized, ability to multi-task, analytical, time management.
  • Ability to work well under stress and provide emotional support to patients and families.  
  • Must have experience teaching individuals and groups, primarily adults.

PREFERRED:

  • BSN or BS with a major in Nursing
  • 8 years of experience in specialty area to include direct patient care and clinical educator experience.
  • Demonstrated ability to guide and influence others.  

Licensure/Certification Requirements

REQUIRED:

  • Current licensure as a Registered Nurse in New York State. 
  • Current Basic Life Support and Advanced Life Support Certifications.
