Supervisor - Social Work - Full Time Days

Job Summary

The Supervisor Social Work is a working supervisor who coordinates and oversees the daily work assigned social work staff members.  The Supervisor will also carry a case load and provide social work services across various care settings. This role involves assessing patient or outpatients’ needs, developing treatment plans, and providing therapeutic interventions.  The Supervisor will adapt their approach to meet the unique needs of each patient and collaborating with other healthcare professionals to deliver holistic care.  

Core Job Responsibilities

  • Handle scheduling, distribute case assignments or instructions, and monitor work for quality, completeness, and client/patient satisfaction.  Promote effective teamwork throughout the assigned area or function.
  • Ensure staff have the resources, tools and training necessary to fulfill their duties.  Responsible for orienting and training new hires.  
  • Supervise assigned Mental Health Counselors, LMSWs, MSWs, students, residents, and unlicensed staff within scope of license.   
  • Conduct thorough psychosocial assessments to understand patients' mental health, social, and environmental needs.
  • Identify and evaluate patients’ strengths, challenges, areas requiring intervention and barriers to effective care.
  • Make recommendations to provider for treatment options and dispositioning.
  • Develop and implement individualized treatment plans in collaboration with providers, patients, families, and the healthcare team.
  • Provide individual, group, and family therapy to address psychological, emotional, and social issues.
  • Support patients in managing chronic conditions, adjusting to life changes, and addressing psychosocial stressors.
  • Provide crisis intervention and support in various clinical situations.
  • Assist in care coordination across hospital and outpatient settings to ensure continuity of care.
  • Facilitate referrals to community resources, support services, and additional healthcare providers to support patient needs.
  • Advocate for patients' needs and rights within the healthcare system and community.
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure integrated and comprehensive patient care.
  • Participate in care team meetings, case discussions, and discharge/care planning processes.
  • Ensure all social work and counseling documentation is accurate and up-to-date for patient assessments, treatment plans, progress notes, and other required records.
  • Adhere to state and federal regulations, as well as organizational policies and procedures, ensuring compliance in all aspects of social work practice.
  • Provide education and emotional support to patients and families about coping strategies, mental health conditions, and available resources.
  • Offer training and support to staff members on social work practices and psychosocial issues.
  • With the leadership and Human Resources guidance, make recommendations on employee hires, performance reviews, transfers, promotions, salary changes, disciplinary actions, terminations and similar actions. Resolve grievances and staff problems in a fair, timely and consistent manner also in conjunction with HR.
  • Keep leadership updated on issues, problems and progress towards goals.
  • Provide and encourage effective coaching. Promote a positive environment of constructive feedback.
  • Handle payroll and record keeping activities.
  • Contribute to continuous quality improvement initiatives and help enhance social work practices within the health system.
  • Keep current in best practices and trends in Social Work topics and meet CEU requirements to maintain licensure.
  • Perform related duties as assigned.
     

Education/Experience Requirements

REQUIRED:

  • Master's Degree in Social Work (MSW) from an accredited institution.
  • Current and valid New York State licensure as a LCSW (preferred) or LMSW.
  • Minimum of 1 year of clinical experience, with experience in a hospital or outpatient settings.
  • Strong understanding of social work practices, ethical standards, and relevant healthcare regulations.
  • Proven ability to lead, guide and motivate staff.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work effectively within a multidisciplinary team and manage a diverse caseload.
  • Proficiency with electronic health records (EHR) and documentation standards.
  • Ability to handle high-stress situations with professionalism and sensitivity.
  • Ability to travel to other worksites within MVHS.

PREFERRED:

  • Knowledge of EPIC.
  • Experience in a hospital inpatient or outpatient environment.
  • Prior supervisory experience in a related field
     

Licensure/Certification Requirements

REQUIRED:

  • Current and valid New York State licensure as a LCSW (preferred) or LMSW.
