System Support Specialist - Family Medicine Center - Full Time - Days
Job Summary
Under the direction of the Sister Rose Vincent Family Medicine Center (SRVFMC) Office Manager, performs various duties with a focus on team based care in a Patient Centered Medical Home model. This collaborative care environment in turn, supports the education and training of family medicine residents and other providers. This position will also perform daily data capture needed to support PCMH, DSRIP, VBP and various other incentive programs as needed and will work in collaboration with clinic leaders, project owners, care providers and team members to help the practice meet regulatory requirements and quality measures as set forth by the State of New York and other agencies in order to meet the measures and goals for each project. This position also assists patients that are uninsured and those with community service needs and is responsible for any other site related functions directed by the Office Manager.
Core Job Responsibilities
- Coordinates the Home Visit Program in collaboration with the SRVFMC Medical Director, Residency Program Coordinator & Lead Faculty Preceptor
- Maintains current knowledge regarding community services available to patients and assists providers in connecting patients to these services by providing them with the necessary information regarding housing, home care, nutritional programs, etc.
- Coordinates and tracks forms from such agencies as OCDSS, Disability, Worker's Compensation and others. Assists in educating providers and clinical staff on form completion if needed
- Helps nursing staff and patients coordinate transportation if necessary
- Works with Finance to manage SRVFMC purchase orders for office supplies, furniture and equipment; signs for and reconciles received shipment invoices and orders. Maintains G/L entries as required
- Monitors Secure Messaging reports from the RHIOs and other organizations and routes to appropriate providers or staff
- Works with FMC managers to update and maintains templates for provider schedules following all EMR workflows
- Coordinates staff meetings, creates the agenda following PCMH guidelines with input from the Office, Nursing, Residency and Specialty Program Managers as well as the SRVFMC Medical Director and Residency Program Director, records attendance, helps with meeting set up
- Coordinates special events such as Casual For A Cause, Special Jeans Days and various SRVFMC events and functions
- Coordinates requests for building maintenance, safety and repair with landlord and the MVHS Facilities Department, places electronic requests for services needed
- Tracks and assists self pay / uninsured patients by providing them with appropriate contact information to apply for Medicaid Managed Care programs
- Assists with scheduling and registration of patients for the SRVFMC and Specialty Clinics if required. Usually only applies when staffing is down and coverage is needed
- Covers operator's seat if necessary for incoming calls and messages
- Is a Notary of Public to facilitate required documentation for the program and the practice
- Other site specific duties, tasks and assignments as directed by manager
- Works with Population Health team on various incentive programs such as Fidelis to coordinate appointments needed to meet quality measures. Delegates scheduling of appointments required to fulfill these measures to the office representatives and other schedulers
- In coordination with the Nurse Manager, works daily Inpatient, Emergency Room and Urgent Care discharge reports & RHIO communications to help coordinate transitional care appointments with schedulers
- Work assignments related to this position can change. Employee must be willing to be flexible in their job responsibilities in order to meet the value based contracts and other incentive programs changes.
Education/Experience Requirements
Required:
- Associates Degree or 3-5 years relatable experience
- Minimum 3 years experience in medical field
Preferred:
- BS in Health Care Administration or equivalent
- Experience in VBP, Quality Improvement, PCMH and EPIC EHR
Licensure/Certification Requirements
Required:
- Licensed Notary Public or willing to obtain license by end of first year of employment
Preferred:
- PCMH Certified Content Expert
