Job Summary

The Registered Nurse - Home Care promotes and restores patient's health by completing the nursing process: collaborating with Physicians and multidisciplinary team members: providing support to patients, care givers and families. Responsible for assessing, planning, promoting and evaluating quality patient care in the patient’s home. The Home Care Nurse provides teaching to patients, families and caregivers.

Core Job Responsibilities

  • Identify patient care requirements and goals.  Assess, reassess and reevaluate the plan of care as needed.
  • Communicate the plan of care to the interdisciplinary team and follow up on delivery of services.  Refer requests to subject matter experts/decision makers appropriately.  Maintain a collaborative relationship within the care team.
  • Promote patient independence.  Educate patients, caregivers and family on the disease process, medications and self-care skills.
  • Manage medications and controlled substances per policy and regulations.
  • Maintain patient supply inventory in the home.  Anticipate supply needs and place orders when needed.
  • Establish a compassionate environment by providing emotional, psychological and spiritual support. 
  • Provide service excellence and customer service to patients, using compassionate patient-centered care, AIDET and positive service recovery methods to professionally manage patient/caregiver needs and concerns.
  • Comply with regulations, nursing standards and MVHS/VNA policies and procedures.
  • Delegate tasks appropriately.
  • Comply with infection control protocols and use universal precautions. Maintain a safe and clean environment.  
  • Document all patient care services accurately and timely.
  • Participate in in-services and educational events.  Keep current through professional publications, societies and networking activities.  Meet competencies and training requirements.  Contribute to staff development, orientation and training.
  • Perform related duties as assigned.
     

Education/Experience Requirements

REQUIRED:

  • Graduate of an accredited nursing program.
  • 1 year Medical Surgical or ICU experience.
  • Ability to travel to patient homes within the region.

PREFERRED:

  • Bachelor of Science in Nursing from accredited nursing program or higher degree

Licensure/Certification Requirements

REQUIRED:

  • Current New York State licensure as a Registered Nurse.
  • Basic Life Support (BLS) certification.
