Structural Heart Coordinator NP/PA

Job Summary

As a Structural Heart APP and Program Coordinator, the PA/NP will work in an administrative and clinical capacity with collaborating physicians, clinical staff, patients and their families, insurance companies, and vendors, in the planning, evaluation, and coordination of care for our Structural Heart patients across the entire continuum of care, including pre-procedural / post-procedural care in the outpatient and inpatient clinical settings.

    • Ensure effective communication with all parties, delivering superior patient care and experience
    • Coordinates overall clinical program and ensures processes are in place to ensure smooth operation and patient flow for Structural Heart patients referred to the Valve Clinic
    • Coordinates the clinical interdisciplinary team and services for pre-procedural evaluation prior to Transcatheter Aortic Valve Replacement (TAVR), Mitraclip and Watchman procedures.
    • Participate in multidisciplinary meetings
    • The role of the Structural Heart PA/NP will evolve over time to meet the needs of the patients and of the Structural Heart Program to include other structural procedures.

Serves as expert clinician resource for the Structural Heart Program at MVHS. Delivers skilled care and experience through the development, implementation, education, collaboration, and evaluation of optimal critical care pathways for the Structural Heart patient population. Serves as education and clinical resource for the med/surg and critical care nursing units in the coordination of care for patients and families.

 

Schedule:

  • Monday through Friday.
  • Physician Assistant shall not have any regular on-call or weekend work requirements.

Core Job Responsibilities

 

  • Coordinate direct implementation of care plans specific to the Structural Heart patient needs.
  • Model excellence in clinical practice through skilled experience in coordination, assessment, planning of clinical care pathway for the Structural Heart patient population. Identify needed co-morbid conditions and obtain testing to satisfy risk analysis and scoring in keeping with the Society of Thoracic Surgeons (STS) organization calculator.
  • Perform patient rounds on Structural Heart patients pre and post procedure. Assess health status through collection, analysis of patient data and evaluate appropriateness of care.
  • Coordinate; implement educational information, materials, and programs for discharge planning needs to Structural Heart population.
  • Implement evidence-based clinical practice enhancements into the Structural Heart patient pathway resulting in enhanced outcomes of clinical and cost effective care.
  • Assess and implement medications, policies, and documentation to satisfy STS, NYS DOH quality measures/metrics. Identify areas of improvement and recommend pathways to improve compliance and satisfy' goals of meeting expected quality measures.
  • Maintain an office and hospital schedule consistent with the needs of patients and the scope of the duties to be performed, which shall include sufficient time for administrative functions.
  • Performance of such administrative functions as Medical Center may reasonably require.
  • Participate and cooperate in any quality assurance, internal peer review, utilization review and external audit programs maintained by Medical Center.
  • Comply with process improvement policies, procedures and initiatives applicable to the Medical Center's surgical service and the Medical Staff's Department of Surgery.
  • Participate in and comply with any billing, coding, corporate compliance or related programs and policies.
  • Attendance at and participation in Heart Team meetings.
  • Other duties as needed to fulfill the mission of Medical Center and the MVHS Medical Group.

Education/Experience Requirements

REQUIRED:    

  • Master’s degree from an accredited Nurse Practitioner's Program OR certification from an accredited NP program OR completion of an accredited Physician Assistant Program required.
  • Ability to meet and maintain credentialing requirements and hospital privileges.
  • Knowledge and competency in a wide range of clinical areas. 
  • Familiar with EHR/EMR software.   

 

Licensure/Certification Requirements

REQUIRED:    

  • Current NYS Nurse Practitioner licenses OR Current NYS license as a Physician Assistant with prescription privileges.
  • Current BCLS, ACLS certifications.
