Survivorship Program - Advanced Practice Provider


Provides leadership and operational oversight to survivorship program and support services. Works collaboratively with Cancer Services leadership to oversee the strategic planning, development and management of the Cancer Survivorship Program. Primary focus is to provide leadership in promoting health, wellness and improved quality of life for cancer survivors during and after cancer treatment by assisting front-line personnel in achieving the highest level of patient safety and improved outcomes through team building, process improvement, and planning, implementing and overseeing supportive care interventions. Utilizes the core tenets of Cancer Survivorship: (1) prevention and detection of new cancers and recurrent cancer; (2) surveillance for recurrence or new primaries; (3) Interventions for long-term and late effects from cancer and its therapies; and (4) coordination between specialists and primary care providers to ensure that all of the survivor’s needs are met.  The advanced practice provider practices autonomously and in collaboration with other health care professionals to diagnose, treat and manage health problems.


Master's degree in Nursing and graduate from an accredited Nurse Practitioner educational program  OR

Master's Degree from an accredited Physician Assistant Studies training program



Current State of New York Nurse Practitioner license, Nurse Practitioner certification from a National certifying body either as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary  Care NP (AGPCNP)

Current State of New York Physician Assistance license, Current certification from the National Certification Commission for Physician Assistants (NCCPA)
